This patient in the late 20s was looking to sneak into the D cup range. She was wanting a bit more upper pole fullness. With her rice test she decided on 350cc, so we placed a 360cc implant on the right and 340cc on the left, Allergan style SSLP, through the under arm using an endoscope and a Keller funnel.

This 44-year-old female was thinking about implants long before having children and once she was done with kids, decided to move ahead. Because she was looking for a little more fullness in the upper pole of the breast, she decided on the highly cohesive silicone. With the rice test we chose 485cc on the right at 445cc on the left with Allergan style SCM silicone gel implants.
As with most of my patients, this 25-year-old was looking for a very natural result and something in that full C / small D range. We went through the underarm using the endoscope and a Keller funnel. The patient chose 385cc silicone gel implants Allergan style SCF after doing the rice test… Her result has a very natural upper pole shape and a nice, balanced breast!
Although I am not usually a fan of high-profile implants in a first-time breast augmentation, her breast base width diameter was only 11.5 cm and after doing the rice test she wanted 345cc. So, we settled on an Allergan style SSF silicone gel implant through the under arm with a Keller funnel. With good placement, even a high-profile implant does not have to look “too high”.
I think it is possible for first time breast augmentation patients to go to a good size high profile implant and still not look way over done. This patient wanted a Victoria’s Secret DD cup size but didn’t want to be too top-heavy. She started with a very good breast shape and we placed 450cc Allergan style SSF high profile silicone gel implants through the under arm using a Keller funnel.

Another good example of getting a larger size implant in through the underarm. The Keller funnel really makes it so much easier than the old days when we had to use our fingers! With the rice test she decided on an Allergan style SSF 485cc silicone gel implant. I decided on a high-profile implant because of that larger size we didn’t want her being too full on the side.
This patient had a very short inferior pole at the bottom of her breast. She also had a fairly wide space and prominent breast tissue. In these cases when going through the under arm, I exit the muscle a little sooner to allow the implant to fill out the bottom of the breast. We did 400cc Allergan style SSLP silicone gel implants using the Keller funnel
In most primary breast augmentation patients, I am not a fan of high-profile implants. For the first time patient, many times I find them too narrow and over projected. However, this patient desired a larger size and had a fairly narrow rib cage. Doing the race test, she liked 550cc so we did place Allergan style SSF silicone gel implants 560cc (yes through the underarm using a Keller funnel!) It is possible, and with a great result.
