It’s sometimes fun to see follow up breast augmentation five years later. This young woman underwent transaxillary breast augmentation with Allergan 371 cc silicone gel implants, wanting to be a full C cup.

I think this patient is a great example of the importance of picking the correct width of implant for each individual case. She presented wanting to be a Full C Small D and had a Base Width Diameter of her native breast of 13-13.5 cm and with just a slight outward tilt to her nipple/areola. She also had a fair amount of her own breast tissue, so a large implant was not needed. With the "Rice Test" we settled on 320cc and rather than jump to a High or even Moderate Profile implants, we settled on a 320cc Allergan Style Low-Plus (SRLP) with great results and great cleavage.
This 21-year-old female is a good example that it is still possible to obtain a very natural result using saline implants and the challenge of a wider space in the cleavage area. She has a slight outward orientation with very little natural cleavage. By not over dissecting the lateral pocket in patients like this, a balanced breast can still be achieved. We placed Allergan style 68MP Saline implants through the under arm using the endoscope 390cc filled to 420cc each side.
Is 38-year-old female started with a fair amount of breast tissue and a very distinct inframammary fold. But she is a good example that this still can be pushed out through the under-arm technique if the tissue is not too dense. We placed 360 mL Allergan style SRLP smooth silicone gel implants through the under arm using a Keller funnel. Postoperatively her upper pole has a very natural slope and good fullness underneath.
This patient is a good example of obtaining a natural breast balance even with a large implant. This patient desired to get into the DD range and through the under arm and using a Keller funnel we placed 485cc Allergan style SRM silicone gel implants on the left and 445cc on the right. By opening up the lower pole of the breast it allows the implant to settle into a very natural position, well balanced underneath the nipple.
I think this patient is a good illustration of when breasts have a different shape from side to side, that difference can become less apparent after implant surgery. She was hoping for a small C and after the rice test we settled on 300cc Allergan style SRLP implants placed through the underarm. A natural result with still some difference but less noticeable.
This patient is a great example of how even when the lower pole of the breast is short, it can still be expanded with the under-arm technique. The bottom crescent of her right breast is a bit more constricted than her left. With the rice test she chose Allergan style SRM 445cc silicone gel implants, which we placed using the endoscope and a Keller funnel through the under arm. Her eight-month postoperative pictures show her well expanded and a great size!
I think this patient is a great example of it’s never too late to get what you want! This is a 53-year-old female who had been thinking about this for years. She now felt it was her turn to do something for herself. She felt her breasts over the years had developed a deflated look and she was hoping to get into a full C small D cup. Because she had a slightly wider rib cage we went with Allergan low-plus profile implant 400cc placed through the underarm and under the muscle, using a Keller funnel. She was extremely happy and has a very natural result!
This 33-year-old with 2 kids is a great example of how a natural result is possible if there exists just a bit of droop pre-op. Technically her nipple is above her crease under her breast, but she does have a bit of relaxed tissue. The implant was placed through the underarm and muscle, but by using the endoscope, the pocket can be developed by exiting the muscle higher up, letting the implant better fill out the tissue. Allergan SRLP 360cc was placed on the right and 340cc on her left.
